    [IsApproved] [bit] NOT NULL,
 CONSTRAINT [PK_GuestBook_EntryID] PRIMARY KEY CLUSTERED
(
    [EntryID] ASC
)WITH (PAD_INDEX  = OFF, STATISTICS_NORECOMPUTE  = OFF, IGNORE_DUP_KEY = OFF, ALLOW_ROW_LOCKS  = ON, ALLOW_PAGE_LOCKS  = ON) ON [PRIMARY]

So there was no specific red error text? Strange. Usually DNN gives a very clear message as to what's wrong. If you are using the email form pattern, it assumes that there should be a field with "email" in it somewhere. The first place I'd check is in the code-behind files, but even with syntax errors, the manifest should have imported. I'll try to reproduce your error, and if you don't mind, send me a copy of your code (including the app_code directory) to patrick at driscollart dot com.
